What this log includes
Each row on the printed sheet has six fields: Date (with optional pre-fill), Time of reading, Systolic (the top number), Diastolic (the bottom number), Pulse, and a free-text Notes column for any context you want to record — such as whether you had just been active, or whether you took the reading sitting or standing. There is also an optional name line at the bottom so the sheet can be identified if you print multiple copies.
How to use this blood pressure log
- Choose your row count — 20 rows fills a standard page with Large Print on; use 25–30 for a more compact sheet covering more days.
- Pre-fill dates (optional) — enter a starting date and the log will label each row for you, so you only need to write in the time and readings.
- Print the sheet — click the Print button or press Ctrl+P (⌘+P on Mac). The log fills the printable area automatically; select your paper size in the print dialog.
- Fill it in by hand — after each reading, write the numbers in the appropriate row. Large-print rows are sized so there is plenty of space.
- Bring it to your appointment — a printed record gives your care team a full picture of your readings over days or weeks in a format that is easy to review together.
Practical tips
- Keep the log and your blood pressure monitor in the same place — near a bedside table or on the kitchen counter — so filling it in becomes part of your routine.
