What this log includes
Each row has five fields: Date (with optional pre-fill), Time of the reading, Reading (the blood glucose value from your meter), an optional Before / After Meal column, and a free-text Notes field for any other context you want to capture. You can choose 10–30 rows per sheet and turn Large Print on or off to suit your preference.
How to use this blood sugar log
- Choose your settings — select how many rows you want, turn on Before/After Meal if relevant, and optionally pre-fill dates.
- Print the sheet — click the Print button or press Ctrl+P (⌘+P on Mac). Select your paper size in the print dialog.
- Fill it in after each reading — write the value and time in the appropriate row. Include the meal context if that information is useful for your record.
- Use the Notes column — note anything that might be useful context, such as physical activity, a missed meal, or feeling unwell.
- Bring it to your appointment — hand your care team a complete written record rather than trying to recall values from memory.
Practical tips
- Keep the log sheet next to your meter so you can fill it in immediately after each reading — it takes only a few seconds.
- If you test at the same times each day, pre-filling the dates saves time and keeps the sheet tidy.
- Clip completed sheets together by month so you have an organised record ready for appointments.
